Jack up the rear end to take as much tension off of the shocks as you can.
Find a cheater bar that will slip over the end of the tool to increase your leverage.
Put the tool's lug in one of the notches in the adjuster, bracing the rest of the tool on the shock, and pull.
Change direction depending on whether you're increasing or decreasing preload.
If you don't care about scratching up the adjuster a little, you can use an adjustable wrench.
